Making an application for information
To access information not available online through our Publications Scheme, you will need to submit a formal application under the Right to Information Act 2009 or the Information Privacy Act 2009.
Your application must:
- be in writing in the approved Information Access Application (PDF);
- provide an address to which a notification of the decision may be sent;
- be addressed to the Business Information Manager on the address below; and
- include payment of an application fee of $39 (cheque or postal order) if the information you request is not related to your personal information.

Amending personal information
Under the Information Privacy Act 2009 (IP Act), a person has a right of access to their personal information and/or amend their personal information if they believe the information to be inaccurate, incomplete, out of date or misleading.
To amend your personal records, you will need to submit a formal application under the Information Privacy Act 2009.
Your application must:
- be in writing in the approved Information Privacy Personal Information Amendment Application (PDF);
- provide an address to which a notification of the decision may be sent;
- specify the particular information you wish to have amended;
- give details as to why the information is believed to be incomplete, incorrect, out-of-date or misleading;
- describe the amendments you wish to have made;
